We would expect you to treat all members of staff with respect. We expect you to keep the practice informed of your current contact details, fill out appropriate paperwork when requested and advise us as soon as possible if you are unable to keep an appointment.
Please note that our patient forms are sent out digitally up to one week before your appointment within your appointment reminder email. Please ensure these are completed before your arrival.

New Patients are required to provide the following information when booking their first appointment:
- Title
- Forename
- Surname
- Sex
- Date of Birth
- NHS Number
- Address (including postcode)
- Home Telephone Number
- Mobile Phone Number
- Email Address
We expect you to keep the practice informed of changes to your contact details, these include changes to:
- Title

- Next of Kin Contact Details
We may ask to see evidence of change of details.
If you do not provide us with up to date contact details we will not be able to provide complimentary recall and appointment reminders. Any referrals sent without suitable contact details will be invalid and returned, this may cause delays to your treatment and treatment plans may require alteration if treatment is delayed.
Forms
We ask that patients arrive a minimum of 5 minutes prior to their appointment times as you will be required to fill out paperwork prior to assessment.
You will need to bring:
- Reading Glasses (if required)
- A list of all current Medication
- Proof of exemption from dental charges (if applicable)
New NHS patients will be required to fillout:
- An NHS Consent form (if you are an NHS patient) – See below for further information
- A Full Medical History – See below for futher information
- A Consent to Contact form – this is a GDPR Requirement and consists of the following:
- Consent to speak to names friends/family members regarding your care
- Consent to contact you regarding marketing
- Preferred Contact Methods
At each assessment appointment you will be required to fillout:
An NHS Consent form (if you are an NHS patient) – this form consists of the following sections:
- Patient information
- Patient Declaration (your consent to be treated and charged under the NHS)
- Ethnic group
- Approval to be contacted about your treatment by the NHS
- Claim for free or reduced cost treatment under the NHS (exemption)
A full medical history including:
- Contact Details
- NHS Number
- Next of Kin
- Doctors Details
- Current Medication
- Past and Current Medical Problems
- Past and Current Medical Treatments
- Smoking and Alcohol consumption
Refusal to complete the forms listed above will result in your clinician being unable to examine and/or provide treatment.
